Comments

These IDs must be the approved, full federal waiver ID number assigned during the state submission and CMS approval process. The categories of demonstration and waiver programs include:

1915(b)(1);
1915(b)(2);
1915(b)(3), and 1915(b)(4) managed care waivers;
1915(c) home and community based services waivers;
combined 1915(b) and 1915(c) managed home and community based services waivers and 1115 demonstrations.
