Home Health Outcome and Assessment Information Set

The Home Health Outcome and Assessment Information Set (OASIS) contains data items developed for measuring patient outcomes for the purpose of performance improvement in home health care. OASIS assessments are collected at specified time points for adult (age 18 or over) Medicare and Medicaid patients. The assessments are required of all home health agencies certified to receive Medicare and Medicaid payments. Note that the Medicare Prescription Drug Improvement and Modernization Act of 2003 (MMA) suspends required completion for non-Medicare and non-Medicaid patients.

OASIS assessment instrument includes:

  • socio-demographic
  • environmental
  • support system
  • health status
  • functional status
  • health service utilization characteristics
