Long Term Care Minimum Data Set 3.0

The Long Term Care Minimum Data Set (MDS) is a standardized, primary screening and assessment tool of health status which forms the foundation of the comprehensive assessment for all residents (regardless of payer) of long-term care facilities certified to participate in Medicare or Medicaid. The MDS contains items that measure:

  • physical
  • clinical
  • psychological
  • psycho-social functioning
  • life care wishes

The years listed reflect the most recent full year available through Buccaneer. MDS data from CMS can typically be requested within three months of the target date. The format of the MDS changed from 2.0 to 3.0 on 10/01/2010. The current version of MDS 3.0 (Version 4/1/2012) is effective for assessments with a target date of 4/1/2012 forward. Assessments with a target date of 10/1/2010 to 3/31/2012 will be output using MDS 3.0 Version 10/1/2010. Note: This means that data from the 4th quarter of 2010 thru the 1st quarter of 2012 will be output in MDS 3.0 Version 10/1/2010 format.
